If telephoning a prescription request into the surgery, please ensure that you use the direct telephone line for your Doctor. This is listed at the bottom of your tear-off slip. When ordering you must have the prescription order slip with you. The Receptionist will ask for your name and verify your address and Date of Birth. You should then state the name, dosage and quantity of medication that you need.
Regretfully, if you do not have these details to hand, the Receptionist may not be able to deal with your request over the telephone. To make things easier for yourself and the Receptionist, please try to order all medication that is due for renewal at once.
Please remember to only order medication that is needed and inform your Receptionist if you have stopped taking any tablets that may be listed on your repeat slip.
Below is a copy of the tear-off prescription request sheet which you should use at all times when requesting repeat medication:
