Blood Tests/X-Ray Results

Before making an appointment with your Doctor to receive any blood test results or x-rays, you should firstly telephone your Receptionist to see if the results are back. Most results can be given over the phone without an appointment to see your Doctor

Change of Name/Address/Telephone Number

If you change your name, address or telephone number please notify your Receptionist as soon as possible, so that your medical records can be updated.

It is important that we have precise and accurate details of all our patients. If you are under the care of a hospital consultant, it is your responsibility to inform them of your change of address etc. Failure to do so may result in you missing an important appointment or hospital admission date.

If changing address you should be aware that if you move out of the practice boundaries you will be asked to register with another Doctor within that area.

Cognitive Behaviour Therapist

Our Cognitive Behaviour Therapist is available to help patients experiencing anxiety disorders, phobias, depression and other problems. Your Doctor will refer you if necessary.

Community Matrons

In 2006 The Dept of Health recognised that people with a serious long term condition or a complex range of conditions, need extra assistance. At Queens Park we have Community Matrons working with us to offer this service. Both are highly experienced senior nurses who can work closely with yourself and your Doctor to plan and organise your care. They will also act as your 'case-manager', being the single point of contact to co-ordinate your health and social needs, or provide support and advice for you and your family.

Community Nurses

Our team of Community Nurses provide nursing assistance for patients confined to their homes. They are based at the Link Centre attached to our building and collect any messages or calls from the Receptionists several times a day.

Community Psychiatric Nurses

You will be referred to one of our CPNs by your Doctor. They provide specialised advice to patients and families in their homes and are based at Clifton House in Stockton.

Counsellors

The practice has two BACD (British Association for Counselling and Psychotherapy) accredited counsellors who provide short term therapy. Counselling can help a wide range of problems.

There are times for most of us when we find it difficult to cope with life, i.e., memories from the past, painful events in the present and worries about the future. You will be referred by your Doctor if it is thought the service will be of benefit to you.

Dietician

A hospital dietician holds a clinic every fortnight at the surgery for newly diagnosed patients with Diabetes. You will be referred by your Doctor or a Practice Nurse to see the dietician to obtain advice on all dietary issues.

Health Visitors

Your Health Visitor will be allocated to you depending on the area you are living in.  If you are unsure who your health Visitor is please contact your Receptionist who will be able to provide you with a name and contact telephone number.  Your Health Visitor will carry out child development/surveillance and is available to offer help and advice regarding family issues and may also be able to prescribe certain medication for you.

Home Visits

If at all possible patients are requested to see the doctor at the surgery where the diagnostic facilities are available to assist in treating you. Home visits are for patients who in the doctor's opinion are too ill to come to the surgery. If you require a Doctor to make a home visit for yourself or a relative, please contact the surgery before 10.00.am. giving as much information as you can to the Receptionist. You may be asked to telephone back at a later time when Doctor has finished surgery so that he/she may speak to you if necessary. Doctors will usually visit over the lunch-time period.

Midwives

You will be allocated a Midwife depending on which area you are living in. She will visit you at your home and will provide ante-natal and post-natal care and offer advice and details of relaxation classes and parent craft.

Should you need to contact a Midwife, your Doctor's Receptionist will be able to provide you with the correct telephone number.

Please inform your Doctor's Receptionist on confirmation of a positive pregnancy test.

Nursing Team

Our nursing team is made up of Nursing Sisters, Treatment Room Nurses and Nursing Assistants.

Practice Nurse Appointments: Many patients are happy to see our nursing team instead of using a Doctor's appointment for the following:

  • Repeat Contraceptive Pill
  • Repeat Depo Provera Injection (injectable contraception)
  • Coil Checks
  • CASH (Contraceptive Care & Sexual Health)
  • Swabs
  • Repeat Hormone Replacement Therapy
  • Blood Pressure Checks
  • Smoking Cessation
  • Eczema Care

Treatment Room: Treatment Room duties include wound dressing, ear syringing and injections. Minor surgery is also carried out.

Practice Pharmacist

At Queens Park we have two Pharmacists who act as a practice advisor. Their job is to advise the Doctors on drug therapy and medicinal issues. They may sometimes ask to see patients to discuss their medication.

Telephone Advice

In some circumstances your Doctor may be able to help you with a problem over the telephone. After you have explained your problem or query to the Receptionist, you will be asked to phone back at a certain time when Doctor has finished surgery, (usually between 11.00 and 11.30am).

Your Doctor will then either leave a message for you with the Receptionist or will speak to you over the telephone. This will save both Doctor's time and your own.

All advice given will be recorded in your computer notes.
